Education Requirements

Students in any major must meet the education requirements (complete one of the following four course before leaving on the internship) in order to become a candidate for the SEED Internship.
  • MSLE 3510 (3): New Venture Creation *(typically the route taken by non-business students)
  • MSLE 3530 (3): New Venture Marketing
  • MSLE 3540 (3): New Venture Finance
  • MSLE 3580 (3): New Venture Execution




Additionally, students that are accepted into the SEED program will be required to enroll in *MSLE 3590 the semester prior to their internship (Interns selected for summer and fall internships will register for the class during the spring semester.  Interns selected for spring internships will register for the class during the fall semester). 

 

*The content of this course focuses on training SEED interns in the following areas: Teaching skills and principles used in effective mentorship and small business consulting, understanding the scope of work and the day-to-day activities during the internship, learning about SEED’s in-country partners, discussing travel/health requirements, and understanding the program’s expectations and guidelines.

Huntsman School of Business - Global Program Participation Agreement

 

I, in consideration for the opportunity to participate in the Utah State University (“USU”) international travel program (“Program”) for which I am applying, hereby agree to:

 

Medical Conditions

(a) Inform the Huntsman Office of Global Learning and Program leaders of any and all of my medical conditions, including, but not limited to, injuries, allergies, medicines, etc., that may need special attention during my participation in the Program;

(b) Be personally responsible for and obtain appropriate medical treatment for any such medical conditions as needed;

(c) Understand the physical, mental, and emotional requirements of the Program; and

(d) Ensure that I am physically, mentally, and emotionally capable of participating in all aspects of the Program.

Vaccinations/ Medications

Obtain any vaccines or medicines recommended to “all travelers” and “most travelers” by the Centers for Disease Control and Prevention (https://wwwnc.cdc.gov/travel/destinations/list).

Insurance

(a) Be responsible for my travel insurance while abroad.  I understand that USU-arranged travel insurance covers the Program dates and may cover up to an additional seven (7) days of travel directly before and/or after the Program dates depending on the needs of the Program and associated travel arrangements; and

(b) Obtain additional travel insurance personally if I desire to have travel insurance before or after the USU-arranged travel insurance coverage.

Student Status & Account

Ensure that my student status is in good standing and clear any holds on my student account.

Academics

(a) Complete my academic course work associated with the Program and fully engage academically in the Program; and

(b) When applicable, complete the Advance Credit Approval Form to transfer credit back to USU.

Passport

Obtain or possess a passport that is valid for at least six months following the conclusion of the Program

Visa

Obtain any required visa(s) for the Program destination country/countries.

Travel Risks & Safety

(a) Be personally responsible for my own health and safety during my participation in the Program and any associated additional personal travel;

(b) Educate myself about current social, political, health (e.g. immunizations, diseases, or other public health issues), and safety conditions and risks of the country/countries to be visited during the Program; and

(c) Complete and activate my International SOS emergency record (https://risk.usu.edu/international_travel/international-sos) or other relevant insurance associated emergency record.

Airfare & Travel Expenses

Be solely responsible for any airfare or other travel expenses that I incur. I understand that such expenses are not eligible for reimbursement by USU and acknowledge that USU recommends that I purchase refundable or changeable airfare to provide for unforeseen Program changes or travel adjustments.

Fees

Pay all Program fees (i.e. application fees, deposits, tuition, etc.) by the prescribed deadlines. I understand that all paid Program fees are non-refundable.

Financial Aid

When applicable, comply with financial aid instructions and deadlines.  If applicable, I authorize USU to apply any financial aid funds at USU’s discretion for all or part of the Program fees due.  I understand that it is my responsibility to sign all necessary documents related to financial aid and arrange for the disbursement of these funds prior to my departure.

Policies & Conduct

Comply with the USU Student Code and all applicable USU policies, including, but not limited to, USU Policy No. 536, International Travel Policy.  I agree that my conduct during my participation in the Program will reflect prudent safety, personal responsibility, cultural courtesy, and academic integrity in keeping with the Student Code, USU Policies, local laws, reasonable international travel practices, and any Program specific requirements.

Photographs

Permit USU to (i) take, record, use, and publish photograph(s)/video(s) of or including me in any manner and medium; and (ii) alter, edit, or manipulate the same photograph(s)/video(s) without restriction, my inspection, or my approval.

Information Sharing and Communication

(a) Permit USU to release my application and other information about my participation in this Program to cooperating institutions, Program leaders, and/or other relevant parties;

(b) Permit USU to share my contact information with other Program participants; and 

(c) Keep my contact information (including preferred email address) current in my student account (Banner) and to regularly check my email for Program information, updates, etc.

Program Alterations

Adhere to any Program activity alterations as may be deemed necessary by USU.

 

I understand and agree that my failure to adhere to the above commitments may result in my removal from participation in the Program.  MY ELECTRONIC SIGNATURE INDICATES THAT I HAVE READ THIS ENTIRE AGREEMENT AND AGREE TO ALL THE TERMS SET FORTH HEREIN.

INTERNATIONAL TRAVEL WAIVER AND RELEASE AGREEMENT

 

I, in consideration for the opportunity to participate in the international travel activity (the “Activity”), for which I am applying, on behalf of my heirs, assigns, personal representative, and estate, hereby agree as follows:

 

ACCEPTANCE OF RISKS AND RESPONSIBILITY:  I acknowledge that the Activity is inherently hazardous and my participation in the Activity necessarily includes certain risks which may result in loss or damage to property or injury, illness, or death to myself or others.  Travel risks include, but are not limited to, vehicular (airplane, automobile, train, boat, bus, mass transit, etc.) malfunctions or accidents; malfunction of equipment; unsafe, dangerous, or defective premises or travel areas; political or civil unrest; natural disasters; and disease, epidemics, pandemics, and various health hazards or dangers.  I understand that these risks may be known or unknown, identified or unidentified, and anticipated or unanticipated.  I also acknowledge that loss or damage to property or injury, illness, or death to myself or others may result from the negligent acts of Activity participants or organizers (including Utah State University, its agents, officers, employees, devisees and assigns (hereafter collectively, “USU”)).  I voluntarily accept the risks and dangers associated with the Activity.  I agree that I am personally responsible for being aware of and complying with travel alerts and/or travel warnings issued by the United States Federal Government or the government of the area to which I am traveling. I agree that I will be personally liable for any damages that occur as a result of my participation in the Activity.  During my participation in the Activity, I agree to adhere to appropriate safety standards to ensure the safety of myself, others, and associated property.

 

RELEASE: I hereby voluntarily release and forever discharge USU from any and all liability, claims, demands, attorney fees, actions, or rights of action which are related to, arise out of, or are in any way connected with my participation in the Activity, including specifically, but not limited to, the negligent acts or omissions of USU for any and all bodily injury, death, illness, disability, or damage to myself or to my property.  I agree that USU is not liable for risks, negligent acts, or any other liability that may arise in connection with my participation in the Activity.

 

INDEMNIFICATION: I hereby agree to hold harmless and indemnify USU against any and all losses, liabilities, or claims arising out of or connected to my participation in the Activity.  The indemnification obligations outlined herein extend to any and all costs and expenses, including attorney fees, incurred by USU in connection with any such loss, liability, or claim.

 

GOVERNING LAW AND JURISDICTION: The laws of the state of Utah shall govern the validity, construction, and enforceability of this Agreement, without giving effect to its conflict of laws principles.  All suits, action, claims and causes of action relating to the validity, construction and enforcement of this Agreement.

 

ACKNOWLEDGEMENT OF EFFECT OF THIS AGREEMENT: I understand and acknowledge that by signing this Agreement, I am agreeing to be bound by its terms and conditions.  I warrant that I am at least eighteen (18) years of age and have authority to enter into this Agreement. I acknowledge that by this Agreement, I have freely and voluntarily (a) given up certain legal rights and/or possible claims that I might otherwise assert or maintain against USU, including specifically, but not limited to, rights arising from or claims for the negligent acts or omissions of USU; and (b) assumed responsibility and legal liability for the claims or other legal demands, including defense costs, which may be asserted by third parties against me as a result of my participation in this Activity. MY ELECTRONIC SIGNATURE INDICATES THAT I HAVE READ THIS ENTIRE AGREEMENT, UNDERSTAND THAT IT AFFECTS MY LEGAL RIGHTS, UNDERSTAND THAT IT WAIVES MY RIGHT TO SUE USU, AND AGREE TO ALL THE TERMS SET FORTH HEREIN.
